4.01 – FAILURE TO NOTIFY SUPERVISOR OR HRD OF CHANGES IN PERSONAL DATA WITHIN SEVEN (7) WORKING DAYS FROM SAID CHANGES
1st offense - oral reprimand
2nd offense - written warning
3rd offense - 1-3 days suspension
4th offense - 3-5 days suspension
5th offense - Termination
4.02 – FAILURE TO DISCLOSE WITHIN FIVE (5) DAYS FROM DISCOVERY OF AFFLICTION, ANY INFECTIOUS OR CONTAGIOUS AILMENT

4.06 – SPREADING RUMORS OR DEROGATORY STORIES ORALLY OR IN WRITING OR IN SOCIAL MEDIA, OR LIBELOUS STORIES/INFORMATION WHICH TENDS TO DESTRUCTIVELY AND/OR NEGATIVELY CRITICIZE, DISCREDIT, DAMAGE OR INTRIGUE AGAINST HONOR, INTEGRITY, REPUTATION OR GOOD NAME OF THE COMPANY, ITS MANAGEMENT OR STOCKHOLDERS OR ITS EMPLOYEES

4.07 – MAKING NASTY, MALICIOUS OR FALSE STATEMENTS OR COMMENTS ABOUT THE COMPANY AND/OR ITS PRODUCTS AND/OR SERVICES IN ANY POSTING ON SOCIAL MEDIA LIKE FACEBOOK/TWITTER AND THE LIKE
