Headset Request

Get Headset Deduction

Agent should get the headset deduction form from HR/ADMIN office afterwards the agent should acquire a signature from the Team Leader or Operation Manager for approval.

Get Headset from HR/Admin Office

After the agents has sign for approval they will go back to HR/Admin Office to get a headset and the agent should fill up the needed details (Name, Date, Campaign, Serial number, and borrowers Signature) It should be made sure that the writings are readable in order for the custodian to encode the details needed properly.

Proceed to IT Department

Once the agent has received the headset from HR/Admin they will proceed IT office for headset checking and remarks make sure the IT staff will sign the form.

Step 4

Go Back to HR/Admin Office

Once the agent has received a remarks from the IT staff, Return the Form to HR/admin Office if the remarks is Good, once if the Headset is defective you will get another headset and let the IT Check it for good remarks.

Step 5

Submit Form

Once the Headset is good and has been signed from an IT Staff. The Agent will submit the form to the Admin or HR if the Admin is not around and will be encoded in admins data.
